Dental anxiety is more common than most people realize. For some, it is a mild unease that passes once they are in the chair. For others, it is a persistent worry that leads to postponed appointments and avoided treatment, sometimes for years. A 2021 systematic review and meta-analysis published in the Journal of Dentistry found that approximately 15% of adults worldwide experience dental fear, with higher rates among women and younger adults. The good news is that most patients with dental anxiety do not need sedation to get through their care comfortably.

At Dental West NYC, we take a patient-centered approach to anxiety that starts with a conversation. Dr. Leora Walter has built her practice around the idea of a genuine dental home, a space where patients feel safe, heard, and unhurried. Because we operate on a fee-for-service model, appointments are never rushed, which means there is time to understand what is driving your anxiety before anything else happens.

Our Approach to Anxious Patients

The most effective tool for managing dental anxiety is often the simplest one: listening. Dr. Walter takes time at the start of each visit to understand where a patient is coming from. Whether the anxiety stems from a painful past experience, a fear of needles, or simply not knowing what to expect, that conversation changes the dynamic entirely. Patients frequently leave appointments noting that what they dreaded turned out to be far less daunting than anticipated.

Ensuring you are thoroughly numb before any treatment begins is equally important. Anxiety and pain are closely linked, and when patients know they will not feel discomfort, much of the apprehension dissolves on its own. If you need more anesthetic, just say so. There is no rush, and your comfort is the priority.

When Oral Sedation May Be Appropriate

For the vast majority of general dental visits, including cleanings, fillings, and laser-assisted exams and cleaning, a thorough conversation and effective local anesthesia are all that is needed. Dr. Walter has found this to be true in nearly every case at the practice.

For patients who are undergoing a surgical procedure and feel they need additional support, a prescribed oral sedative may be an option. This typically involves taking a single dose of medication before the appointment to reduce anticipatory anxiety and promote relaxation during the procedure. It is a targeted solution for specific situations, not a standard part of every visit. Some of the scenarios where this may be discussed include:

  • Oral surgery procedures involving tooth extractions or more involved treatment
  • Patients with a documented history of severe dental anxiety who have not responded to other comfort measures
  • Cases where the scope and length of treatment make additional relaxation support appropriate

If you believe you may benefit from oral sedation ahead of a procedure, that conversation is always welcome and entirely judgment-free.

What Sedation Dentistry Is Not

It is worth being clear about what sedation dentistry at our practice does not look like. We do not prescribe anxiety medication routinely, and we do not believe that every patient who reports nervousness needs pharmacological support. Patients who require general anesthesia or IV sedation for extensive work, such as having multiple teeth removed at once, represent a very different clinical situation, one that involves a separate setting and team. That level of sedation is beyond the scope of a general or oral surgery visit and would be referred to the appropriate specialist.

Our goal is to help as many patients as possible receive the care they need without relying on sedation at all. In Dr. Walter’s experience, when patients are given enough time, enough information, and enough reassurance, they often discover they are more capable than they thought.

Building a Practice Around Comfort

The structure of our practice directly supports how we manage anxiety. Because we are fee-for-service, we are not constrained by insurance-driven scheduling that rushes patients in and out. A new patient who arrives 20 minutes early and wants to sit and talk before treatment begins is welcome to do that. Routine checkups and cleanings become a regular opportunity to build familiarity and trust, which often makes subsequent visits easier for anxious patients over time.

We also offer complimentary fluoride treatments during cleanings, a small but meaningful addition that reflects our commitment to comprehensive, attentive care at every visit.
